Our office operates on the appointment system, taking walk-in patients whenever possible.

Please be on time for your appointment. Please be sure to provide the office with your most current contact information. In the event an emergency arises and the physician must reschedule your appointment, we will contact you at the earliest opportunity.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

Payment is due at the time of service. A service charge of $10.00 will be added to help cover accounting costs if you choose not to pay at that time.

Insurance

It is the patient's responsibility to know his/her insurance benefits. Preventative medicine, physical exams, pap smears, immunizations, etc. are not covered by most insurance policies.

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan.

Benefit information can be obtained from your insurance company or through your employer's personnel department. Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit. In most cases, as a courtesy to our hospitalized patients, our physician's fees for hospital services are billed directly to your insurance company.

If your insurance does not pay in a timely manner, we will expect payment from you, unless other arrangements are made with the office.

We accept Medicare assignment. If you have any questions regarding your insurance or account, please call our insurance and billing specialist at (352) 375-6836.

Participating Insurance Plans

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise. The physicians reserve the right to charge patients a fee for cancellation inside of 24 hours prior to their appointment.

We are happy to help with refill requests during regular office hours, so please call us at (352) 333-6680 to let us know what you need.

After-Hours & Emergencies

Gainesville Family Physicians provides 24-hour access to urgent-care after hours through our on-call services. If you feel you have an emergency that cannot wait until normal business hours you may contact the on-call physician at (352) 333-6680. Press 1 and your call will be automatically routed to the after hours physician on call. You may also dial directly at (352) 395-7256.

For life's minor emergencies that require the attention of a provider after hours you may also seek care at Express ER, a division of the emergency department of the North Florida Regional Medical Center. They are located on the corner of 57th St. and Newberry Road. They are open in the evening until 8pm, on weekends and during the holidays when your regular provider is not in the office.
